Local Number Port Request

This Local Number Porting Application is to be read together with the applicable, Wholesale Voice or ISDN & PSTN Lines Service Order Form and will be governed by the same terms and conditions incorporated by reference in those Service Order forms. A copy of the customers latest invoice must also be attached to the order

Local Number Porting is an important part of the delivery of your SIP trunks or Fixed lines and involves a number of third-party organizations. In order to deliver your service in a timely manner, we ask you to return this form as soon as possible.

Requesting changes from your existing carrier, It is important that you do not make changes to your current service whilst we are porting your numbers or are in the process of delivering your SIP Trunks or Fixed Lines. Often unrelated requests to your current carrier, that do not to come into effect until a later date, can cause delays. If you need to contact your current carrier, please speak with us first to confirm the port process will not be held up, failed Ports can incur additional charges.

By sending this Authorization:

  • I warrant that I am duly authorized to make this application and request the actions set out in this document on behalf of the Company stated above.
  • I warrant that the details stated in this form are correct and accept that any variation to this information may incur additional charges and delays in providing the porting.

I acknowledge that I have been advised that: 

  • By porting the telephone number(s) listed on this form, the service associated with that telephone number is disconnected from the existing service provider’s network and may result in finalization of the account for that service.
  • By porting the telephone number(s) listed on this form, any DSL/Spectrum Sharing service associated with that telephone number is disconnected and may result in finalization of the DSL Spectrum Sharing account for that service; and
  • Although I have the right to port the telephone number(s), there may be costs and obligations associated with the port which may include early termination fees and porting fees.

 I accept that I may only withdraw authority to port this telephone number before the Electronic Cut-over Advice is sent to our current Service Provider, which will be on or after the preferred cut-over date specified in this form

I understand and accept that:

  • Future Technologies provides no guarantee that it can port our telephone number from our current service provider. Our current Service Provider may reject this port request if the information provided is incorrect or does not match the data held by them. In this case we authorize Future Technologies to correct the information and resubmit the request to port our telephone number or dispute the rejection by our current service provider. A porting request may also be rejected for other reasons as stated in the LNP Industry Code.
  • Future Technologies provides no guarantee that the telephone number will be ported within any specified timeframe. Porting hours of operation are 8am to 5pm AEST/AEDST Monday to Friday, excluding national public holidays. Cut over can only be initiated at least 2 business days after the porting Notification Advice is sent by Emerge to our current Service Provider. If a port request is rejected and needs to be resubmitted, cut over cannot take place for at least another 2 business days after the request is resubmitted.
  • In the event of a port, withdrawal or reversal, Future Technologies is not responsible for any period of outage.
  • We may have outstanding contractual obligations and costs owed to our current Service Provider. Future Technologies is not liable for any such costs.
  • Only our telephone number will be transferred to Future Technologies. This may result in the loss of any value-added services that are associated with the service provided by our existing Service Provider (e.g., Voicemail).
  • Future Technologies reserves the right to charge a fee for porting the telephone numbers to or from Future Technologies
  • I understand that Local Number Portability (LNP) does not guarantee that we can keep the telephone number if we move to a different geographic location
